Just my theory and I might get 'shot down' the film RUST is a western set in the 1880's .the American film industry is very fond of using replica firearms which are made by companies such as Pedersoli  their reproduction guns are beauts and yanks can't resist playing with them,They fire real modern ammo available to anybody in places like Walmart.i reckon at least half of the crew have a gun in their car. Someone has 'borrowed' one of the 'prop' guns gone out into the bush loaded it with their real Ammo and had a play with live ammo, maybe they were interrupted and called back to the set before completely  Discharging the real ammo, returned said gun to the armourers table and gone back to work on the set leaving a couple of live rounds in the weapon.

Supposedly even with blanks there is some form of projectile discharged

This can happen. On the news they highlighted the film Crow where Bruce Lees son was killed in just this way.

This incident is unbelievable but America is awash with weapons and ammunition. It's difficult for Brits to comprehend.
